CCP National Events

HealthSciences Institute periodically delivers live CCP events that are open to individual professionals interested in supplementing the CCP online learning program with a live program (blended learning)

  • All participants receive access to the CCP online KnowledgeSpace and Learning Community, and a CCP program and reference manual upon registration.
  • Participants attend the intensive live CCP session in which the imperative for change is established, program content is reviewed, and learning activities facilitated, to build critical competencies for patient partnering, behavior change and health coaching.
  • The live CCP program is typically 1.5 days an is delivered as a preconference or postconference event in coordination with a national conference.
  • Participants complete the online program and certification examination within a year of the original CCP program registration date.

Onsite Organization & Regional Events

The live CCP program is most commonly delivered on-site in specially-designed sessions for health care organizations or regional chronic care improvement collaboratives. HealthSciences partners with organization or program leaders to design an event aligned with the specific organization or program requirements. The CCP program may be offered to nurse staff members in a disease management organization or program, or, to physicians, nurses, pharmacists and other professionals on the interdisciplinary care team. On-site and regional events are available to groups of 25 or more.
