CCP Organization Delivery Options

Since 2004, HealthSciences Institute has partnered with States, health plans, health systems and international organizations to prepare health care professionals and teams for success in the new health care environment.

For our organization partners, the CCP can be delivered to teams of 25 or more through a blended learning format (combining a 1.5 live, onsite session, combined with one-year access to the CCP online KnowledgeSpace and Learning Community). Alternatively, the program can be offered through the CCP online KnowledgeSpace and Learning Community only.

Organizations who require CCP certification for their staff or teams can attain CCP staff accreditation, which includes use of the CCP seal on web sites or in RFPs or proposals.

Partner organizations are eligible for a special 25% reduced tuition and certification fee for their staff.

Many organizations choose CCP certification as a:

  • Pre-approved tuition-reimbursement or continuing education offering.
  • An ongoing new hire orientation program that is supplemented by existing organization-specific content or programs.
  • Required or recommended certification program for organization leaders or staff
  • Onsite CCP learning event that is offered to professionals supporting an organization or regional chronic care improvement demonstration program or initiative.
  • Career ladder or retention tool for key staff.
