News Case Studies Calgary Disease Management ProgramBuilding a Regional Chronic Care Improvement ProgramIn 2006, the Calgary Health Region selected the CCP Certification program for delivery to nearly 100 nurses, advanced practice nurses, pharmacists, nutritionists and kinesiologists through funding from the Alberta Health Ministry. Calgary Health Region has been recognized by the World Health Organization as one of most advanced chronic care collaboratives in the world. The Chronic Disease Management (CDM) Initiative with the Calgary Health Region began in 2002. The focus of the initiative is to develop a regional strategy for improving the management of chronic diseases—including CAD, HF, diabetes, and COPD—across the continuum of care. The program has demonstrated that outcomes across a range of chronic diseases can be improved if a multifaceted approach is taken and attention is given to the community, health system, self-management support, delivery system design, decision support and clinical information systems. Today, the Calgary Health region continues to enroll program staff in the CCP program as key strategy for supporting workforce readiness and chronic care improvement.
