The New Health Care Environment
Health care is changing and chronic diseases are among the biggest threats to health worldwide and account for over 75% of health care spending. In this new health care environment a broader application of evidence-based care standards is the best hope for promoting consumer health, independence and resource management.
New Professional Competencies are Required
Both the Institute of Medicine and the World Health Organization recognize chronic care as a new model of care and that it will require new skills from health care professionals. However, according to a 2007 DMAA and HealthSciences Institute (HSI) national survey, few professionals working in health plan or disease management organizations bring any formal training or experience in chronic care improvement or health coaching to their new roles including.
The Chronic Care Improvement Team
In addition to physicians, nurses and advanced practice nurses, pharmacists, and case managers, chronic care improvement and primary care-based medical home teams may include: behavioral health providers, respiratory therapists, dieticians, rehabilitation therapists, exercise physiologists, as well as complementary and alternative care providers.
Preparing Health Care Professionals for Patient-Centered Care
Designed to augment the specialized training and skills of health care providers, the CCP program is consumer-focused, not discipline-focused. By definition, chronic care improvement requires interdisciplinary teams and a shared vision for improvement. CCP certification translates the patient-centered care vision into practice.
Better care requires team members who can drive evidence-based medical care and apply the new generation of structured, evidence-based partnering, behavior change and health coaching interventions, including motivational interviewing.
Approved for Continuing Education Hours for Health Care Professionals
The program has been accredited for continuing education hours for nurses, physicians and pharmacists. Today, CCP certification is now required or recommended for professionals by leading health care organizations in the US and abroad committed to better patient care and high performance.
