CCP Preconference Event

A half-day CCP preconference event will be offered from 1:00 to 5:00 on Sunday, February 28th at the Philadelphia Marriott in conjunction with the 10th annual Population Health Improvement & Disease Management Colloquium (February 28-March 2), Second National Medical Home Summit (March 1-2), and National Retail Clinic Summit (March 1-2).

In a special arrangement with the conference organizers, this four-hour session, CCP web-based online program and KnowledgeSpace, CCP manual, and certification examination will be offered at the reduced tuition fee of $971.25. Program manual and access to the online program is provided at the time of registration.

The preconference provides a high-level overview of the CCP program content, CCP exam and certification process. Participants complete the online program (before or after the preconference) and certification examination within one-year of registration.

CE hours (40.0 CEs/4 CEUs) are available for nurses, physicians, pharmacists, and case managers who complete the preconference, online program AND examination.

Chronic Care Professional (CCP) Organization & Regional Events

HealthSciences Institute is currently scheduling US and international CCP learning events for 2009 and 2010. Sessions may be hosted for regional or organization delivery to groups ranging from 25 to 250 participants. All sessions are conducted on-site or at a regional location.

All CCP live events are offered in a blended learning format, combining an intensive 1.5 day session, with a web-based program and final certification examination.

CCP onsite events can be designed for delivery to professionals and teams from health plans, health systems, regional collaboratives, or community and provider organizations.

The highly acclaimed live CCP program covers core program content, and includes interactive team components to help participants develop and apply critical health coaching and motivational interviewing competencies.

The Web-Based CCP Certification Program

For individuals or organizations interested in a web-based program, the CCP online KnowledgeSpace features Adobe Flash-based content, with audio, learning activities, and a host of resources to build and apply new competencies--and to successfully complete the CCP online certification examination.

The CCP KnowledgeSpace includes the largest online library of motivational interviewing (MI) video--demonstrating MI in action. The online program can also be supplemented by customized webinar events, to reach remote employees and synchronize the learning process for teams and organizations.
