Participation in the Chronic Care Professional (CCP) Certification program is funded by organizations committed to preparing health care professionals and interdisciplinary teams for patient-centered chronic disease prevention and care improvement. CCP Evaluation Data

The CCP program is a competency-based learning and development program based on the core health care workforce competencies identified by the Institute of Medicine and the World Health Organizations. HealthSciences Institute maintains a firm commitment to demonstrating the return on investment (ROI) of the CCP program in collaboration with sponsoring organizations and third-party evaluators.

The CCP program was first piloted and evaluated in 2004 through funding from the State of Minnesota, in partnership with a Minnesota health plan, county public health units, and providers. The pilot program was recognized for excellence by the Minnesota Department of Human Services in 2005.

In 2007, a national disease management professional development survey was conducted by HealthSciences Institute and DMAA: The Care Continuum Alliance, validating the target competencies and CCP curriculum design.

Independent third-party studies evaluating disease management or health coaching programs that require the CCP program for their staff have demonstrated improved member medical cost avoidance, health behaviors and quality of life measures.

Additionally, evaluation data from individual participants and organization partners has proven key to the success and improvement of the program. Each year, CCP evaluation data is reviewed, and improvements and updates made to the curriculum design and program content. Evaluation data sources include:

  • On-line and post-live session participant evaluation forms
  • Structured post-session and follow-up interviews with participants
  • Structured organization post-session and follow-up interviews

The evaluation process has yielded important qualitative and quantitative data. Highlights of this data follow:  

  • 90% of organizations who offer the CCP program to their professional staff expand enrollment in the program
  • 95% of individual CCP program participants agree or strongly agree that the CCP program has "better prepared" them for their daily job roles
  • 98% of individual CCP program participants agree or strongly agree that the CCP program content is relevant to their professional roles
  • 94% of individual CCP participants agree or strongly agree that the program is a “good value” for their organizations
  • Mean ratings of 5.4 (6.0 scale) have been achieved on all program evaluation domains for both the live and eLearning programs since program inception

In the five years that the CCP program has been offered, over 80% of all tuition fees have been been invested in the continuous improvement and updating of the CCP program and online KnowledgeSpace.
