HealthSciences News Archive2010 2009 2008 2007 2006 2005 2004 2003 Version 2.0 of the Chronic Care Professional Program Released December, 2004. Following the State of Minnesota Chronic Care Demonstration Project, HealthSciences Institute released version 2.0 of the Chronic Care Professional (CCP) Certification program. State of Minnesota Chronic Care Demonstration Project October, 2004. HealthSciences Institute received funding from the State of Minnesota Department of Human Services to deliver the Chronic Care Professional (CCP) Certification program to public health and social services staff from over ten Minnesota counties in partnership with PrimeWest Health Services. Chronic Care Professional Certification Program Announced August, 2004. HealthSciences Institute completed the Chronic Care Professional Certification program in partnership with the HSI advisory committee and national health care stakeholders.
