The Calgary Health Region selected the CCP Certification program for delivery to nearly 100 health care professionals in the Calgary Health Region and other Alberta Health Regions. Calgary Health Region has been recognized by the World Health Organization as one of most advanced chronic care collaboratives in the world. Horizon BCBS of New Jersey Delivers HSI's Coronary Artery Disease Patient Self-Care Products to 30,000+ Members August, 2006. Horizon BCBS of New Jersey selected HSI's Coronary Artery Disease and Heart Failure Patient Self-Care Tools for delivery to over 30,000 members to increase adherence with evidence-based care medical treatment and support patient engagement and self-management.Ontario Health Ministry Funds HSI Program for Toronto East General Hospital System Family Health Team April, 2006. The Ontario Health Ministry funded an HSI program delivered to family practice physicians in Toronto implementing the Province's Family Health Teams (FHT) model of primary care. The FHTs are leading the redesign of Ontario's primary care system through multidisciplinary teams, technology, practice process improvement, and patient self-management support. Province-wide over 250 physicians and 225 other practitioners are participating in FHTs, serving an estimated 500,000 patients.
