HealthSciences News Archive2010 2009 2008 2007 2006 2005 2004 2003 HSI Backs Patient-Centered Primary Care Collaborative (PCPCC) October, 2008. HealthSciences has joined a collaborative of major employers, consumer groups, patient quality organizations, health plans, labor unions, hospitals, physicians to develop and advance the patient centered medical home. The Collaborative believes that, if implemented, the patient centered medical home will improve the health of patients and the viability of the health care delivery system. As a PCPCC partner, HealthSciences will: 1.) Advocate and advise on strategies and solutions for engaging and preparing physicians, nurses and advanced practice nurses, pharmacists, and other team members, for interdisciplinary and evidence-based patient-centered care, 2.) Recognize and support the formal and volunteer agencies in local communities who offer critical support to primary care teams by providing a safety net to patients with chronic or age-related conditions, and their family caregivers. CCP Program Now Accredited for Continuing Education Hours for Physicians and Pharmacists September, 2008. Today's physicians and pharmacists are leading implementation of patient-centered medical home models and integrated care teams to improve chronic care and health care performance. To prepare physicians and pharmacists for success in the new health care environment, the CCP program is now accredited for 40.0 CME and ACPE hours for physicians and pharmacists in partnership with Chicago's Academy for Continuing HealthCare Learning. HSI Partners with Disease Management Association of India (DMAI) to Offer CCP Certification Program

HealthSciences Institute will partner with the Disease Management Association of India (DMAI) to support the application of disease management strategies, knowledge and tools in India--beginning with delivery of the CCP certification program in that country in 2008. Read the press release. HealthSciences Delivers CCP Program to Multidisciplinary CVD Program Team at Kaiser Permanente August, 2008. HealthSciences delivered the Care Professional (CCP) certification program to nurse practitioners, physicians, case managers, dietitians and pharmacists at Kaiser Permanente's Georgia Region in July in coordination with the launch of the region's new Cardiovascular Disease (CVD) Management program. The CVD management program aims to improve evidence-based care delivery, chronic condition self-management, and shared-decision making for members with CVD disease, heart failure, diabetes or hypertension. Read the press release. Chronic Care Professional (CCP) eLearning Certification Site Upgraded and Expanded May, 2008. HealthSciences significantly upgraded and expanded the CCP eLearning program. The new self-directed online program features Adobe Flash Player-enabled learning with audio. Additionally, access to the CCP eLearning program has been expanded from six months to one year. HealthSciences will be adding new learning resources, professional collaboration, and knowledge-sharing features to the site in 2008. Fourth Edition of the Chronic Care Professional (CCP) Program Released May, 2008. HealthSciences released the updated CCP program featuring substantially updated and expanded content on patient partnering, behavior change facilitation and health coaching practice and interventions. Additionally, evidence-based care guideline overviews for over 25 chronic diseases and conditions were updated for 2008.HealthSciences Institute Supports National Sudden Cardiac Arrest (SCA) Prevention Program February, 2008. HealthSciences Institute will deliver education and performance improvement planning sessions to US cardiology clinics and hospitals in several US locations in 2008 to improve prevention rates for SCA. The SCA prevention program is sponsored by Medtronic. CCP Program Offered at the Three Leading Disease Management & Case Management Events in US January, 2008. HealthSciences has agreed to deliver the CCP program as pre and post-conference sessions at the three major DM and case management events in the US.
