Population Health Improvement (PHI) Learning Collaborative 2010

Many of you who work with people at risk of, or affected by, chronic conditions in wellness, disease management and care management settings told us that you wanted a live forum where YOU could share best practices, build critical practice skills and collaborate with peers. HealthSciences Institute and Partners in Improvement are pleased to host the first learning community for front-line clinicians in population health improvement.

Sharing Best Practices & Building New Skills

Each month our community invites an expert or innovator to present on topic, best practice, or skill set in wellness, disease management or chronic care improvement. Each presentation is followed by a 30 minute ask-the-expert segment and discussion.

These live webinar events are FREE of charge, FREE of commercial content, and are NOT vendor showcases. They are open to CCP and non-CCP professionals alike.

Join us on the first Friday of the month (excluding holidays) from 10:30 to 11:30 (CST) for the learning event and PHI Learning Collaborative and CCP community call from 11:30 to Noon (CST).
