News Case Studies Implementing a State-Wide Chronic Care Improvement Strategy in MinnesotaIn 2003, HealthSciences was enlisted by leadership of the State of Minnesota Department of Human Services to develop a state strategy and implementation plan to reduce health care spending and support consumer independence through the application of population-based chronic care improvement. In 2004, the CCP program was first piloted through funding from the State of Minnesota Department of Human Services (DHS) in partnership with Minnesota health plans, health systems, counties and professional associations. In 2005, two of the three state health systems change programs recognized by DHS for excellence were led or supported by HealthSciences Institute (including a health plan-based chronic care improvement program and a primary care-based early intervention program for older adults affected by Alzheimer's Disease and their caregivers). Today, the CCP program continues to be funded by health plans and health systems for delivery to nurses, advanced practice nurses, physicians and pharmacists in health plan, health system, clinic, service groups, and long-term care organizations across much of Minnesota. HealthSciences Institute’s chronic care improvement strategy continues to be implemented by Minnesota DHS staff and serves as a template for Minnesota’s health and long-term care system change grants.
