News Case Studies State of Wisconsin Systems Change:Community Connections to Promote Independent Living (CCPIL)In January 2007, Marshfield Clinic, Security Health Plan and partner counties received a State of Wisconsin grant to deliver HealthSciences’ CCP certification program to build collaboration across private and public systems, and support the delivery of evidence-based care and patient self-management among community-dwelling vulnerable elders with chronic conditions including heart failure. The CCP program was delivered to nurses, advanced practice nurses, and case managers from Security Health Plan, the Marshfield Clinic , Wisconsin counties, the Medical College of Wisconsin, and Security Health Plan. In August 2008, the U.S. Department of Health and Human Service’s Center for Medicare & Medicaid Services (CMS) recognized Marshfield Clinic, for the second time for improving the quality of care while decreasing health care costs in the second of a four year Physician Group Practice (PGP) Demonstration project. Today, the CCP continues to be delivered to professional staff involved in this project. |
