XLHealth: CCP Staff Accreditation for a Leading Disease Management, Medicare Advantage and Special Needs Plan

XLHealth is a Baltimore-based organization, owner and operator of Care Improvement Plus (one of the largest  Medicare chronic condition special needs plan), and a NCQA fully-accredited disease management program. XLHealth also provides a complete Medicare Advantage Part D health plan including disease management services to  Medicare beneficiaries living with heart failure, diabetes, chronic obstructive pulmonary disease and/or end-stage renal disease. XLHealth selected CCP accreditation for their registered nurses and licensed practical nurses to demonstrate the readiness of their professional staff to deliver optimal value to purchasers and patients. XLHealth enrolled their staff in the CCP online program and they are completing the CCP program modules. Once participants complete the online program, they will participate in a live HealthSciences Institute 1.5 day on-site CCP session overviewing CCP program content and building team and individual patient care and self-management support competencies. Staff will complete the CCP national examination following completion of the online and live programs.
