Chronic Care Professional (CCP) Certification Preconference
September 20-21, 2009

A one day CCP session will again be offered as a preconference to the DMAA Forum 2009 in San Diego. Participants will attend the preconference session, and complete the remainder of the program online, as well as the certification examination, at a later date. Chronic Care Professional (CCP) Onsite Events & Online Learning (Ongoing)
HealthSciences Institute is currently scheduling US and international CCP learning events for 2009. Sessions may be hosted by organizations for groups of 25 or more. Improving Your Practice: The Patient Education Trap
Shavaughn Carter is a 43 year old single parent who is obese and was diagnosed with diabetes approximately ten years earlier. She was recently contacted by Melissa Benson, a nurse health coach, with her health plan's Living Well with Diabetes program. After getting acquainted and collecting some initial information, Melissa asks Shavaughn if she knows what she needs to do to manage her diabetes. Shavaughn replies "Well, after ten years I guess I must be an expert by now." So Melissa asks Shavaughn to describe how she manages her diabetes every day. Shavaughn rolls her eyes and thinks to herself "Well, here we go again." At the end of the phone call, Shavaughn agrees to start checking her blood sugar daily again, and they schedule a follow-up call in two weeks to assess progress. Melissa is pleased with the progress that they have made and Shavaughn seems receptive. Following the call, Melissa sends Shavaughn the "Living Well with Diabetes" guide.

Is the Health Care Workforce Ready for Chronic Care?
The January/February Issue of Health Affairs was devoted to the "Crisis in Chronic Disease." Health Affairs Editor-In-Chief observes: "As in many things in health care and health spending, American 'exceptionalism" is the rule: The United States is doing an especially rotten job of delivering chronic care, at spectacular cost." Several excellent articles in this issue address the scope of the chronic disease problem; the value, benefits and limitations of disease prevention and prevention programs, and a current state assessment of disease management and chronic care improvement.

Study Finds That Many Medicare Patients are Rehospitalized, Raising Costs
A study published in the April 1st New England Journal of Medicine found that as many of one-fifth of all Medicare patients are readmitted within a month of being discharged and one-third are rehospitalized within three months. Moreover, half the patients who returned to the hospital within 30 days of receiving treatment (other than surgery) did not see a doctor before they went back, leaving many older patients with heart failure or diabetes to cope on their own. Frequently, these patients do not receive clear instructions regarding which medications that they should take or which warning signs to watch for. However, organizations such as Geisinger have proven that active efforts to reduce readmissions can have a big impact.

New ACC/AHA Guidelines for Heart Failure Released
Improving clinic and hospital adherence with evidence-based guidelines for HF is key to reducing avoidable hospitalization and rehospitalization in the US. On April 15th, The American College of Cardiology and the American Heart Association announced the release of updated guidelines for the diagnosis and management of heart failure.
