Partners in Improvement:Population Health Improvement (PHI) Learning Collaborative 2010
In 2010 you will have the opportunity to join a unique community of professionals who will meet together 'virtually' to share innovations, best practices and success factors in wellness, disease management and care management. Leading health plans and health care organizations will participate in this PHI Learning Collaborative—an ongoing event sponsored by HealthSciences Institute. The Collaborative will feature brief presentations, “ask the expert” segments, and targeted discussion of best practices and solutions in PHI. Guests will include program and thought leaders from health care organizations, universities, and solution providers. CE hours for CCP recertification is available for qualified participants. The first session of the Learning Collaborative will be held on January 8th, 10:30 to 11:30 Central time. We'll be joined by Tina Ross-Knapp, of Capital BlueCross, who will be discussing her experience building their population management programs. On February 5th, 10:30 to 11:30, we'll be joined by Dr. Judith Hibbard who will be talking about the critical concept of patient activation. Judith Hibbard, M.P.H., Dr.P.H., is a professor of health policy at the University of Oregon. Over the last 26 years she has focused her research on consumer choices and behavior in healthcare. Dr. Hibbard is the lead author of the Patient Activation MeasureTM (PAM) and advises many healthcare organizations, foundations and initiatives. Participation is free to individuals currently enrolled in CCP or CCP-certified, and for a limited time, to all subscribers of the HealthSciences Institute eNews.
