US Air Force Health Services Program Selects CCP

Military health service organizations, teams and professionals have been implementing a variety of innovative wellness, disease management and care management programs for members of the armed services. Since 2005, the CCP program has been selected by units and professionals from the Air Force, Army and Navy—where it is now the recommended credential in population health improvement for military health professionals. The Air Force has recently selected the CCP program for national delivery to Air Force nurses serving in disease management and care management roles.
