BCBS of Illinois Hosts Third CCP Conference

Planning is underway for a third Chronic Care Professional (CCP) conference for population health improvement staff at Blue Cross Blue Shield of Illinois (BCBSIL). This session follows two previous sessions with the organization’s disease management and care management staff in 2009. HealthSciences Institute will be on-site at BCBSIL’s headquarters in Chicago to deliver a one day live session delivering the CCP program curriculum and facilitate team activities and demonstrations to build new skills in evidence-based health coaching that support member engagement, activation, treatment adherence and self-care support. The blended program offered at BCBSIL also includes the award-winning CCP online learning curriculum and comprehensive motivational interviewing video library.

Blue Cross Blue Shield is a division of Health Care Service Corporation (HCSC), the largest customer-owned health insurer in the United States and fourth largest health insurer overall. HCSC operates through its Blue Cross and Blue Shield plans in Illinois, New Mexico, Oklahoma and Texas. BCBSIL joins population health improvement professionals from over 20 BCBS affiliates who have selected the CCP program.
