2008 Chronic Care Professional (CCP) Open Sessions

In addition to our ongoing self-directed CCP eLearning and on-site organizational programs, HealthSciences Institute will offer three open sessions of the two-day live Chronic Care Professional (CCP) certification program in 2008 in conjunction with the Disease Management Colloquium, Case Management Society of America (CMSA) and DMAA: The Care Continuum Alliance.

All CCP live programs feature a blended learning approach. Participants attend the live session which overviews foundations of population health improvement and combines active learning experiences to build critical patient partnering, behavior change, and health coaching competencies. Following participation in the live program, attendees complete the self-directed eLearning program and online certification examination, on their own, outside of the session. The entire blended learning program is 40-hours in length

Each of the 2008 programs will be offered at the special 25% discounted tuition fee of $971.25, which includes access to the eLearning program, 40 hours of continuing education credit, a comprehensive program and reference manual, and national CCP certification.
