It’s a New Health Care Environment

Chronic not episodic conditions are the biggest threats to population health, accounting for over 75% of total health care spending in the US. Chronic care improvement will dominate health care in the 21st Century and case managers are uniquely positioned to play key role in this new environment. At the same time, concerns regarding the readiness and supply of case managers for the demand have been voiced. In fact, 80% of respondents to a 2008 national Salary and Trends Survey conducted by Case in Point journal were 46 to 65 years old leading to "growing concern over aging practitioners and void in the younger ranks."

New Roles for Case Managers in Health Plans

Health plans, employer groups, and disease management organizations continue to expand disease management, case management, and health coaching programs and services. To be sustainable, these programs must deliver measurable value to purchasers and patients. Program success depends on case managers who are prepared to drive evidence-based medical care and apply the new generation of structured, evidence-based partnering, behavior change and health coaching interventions, including motivational interviewing.

New Roles in Health Systems, Hospitals and Physician Practices

Public and private health care purchasers and plans are profiling and benchmarking physician practices and hospitals--and public reporting of provider performance and pay-for-performance reimbursement is expanding. The American Academy of Family Physician’s patient-centered medical home and chronic care improvement model is being widely embraced by physicians, payers and health plans. Opportunities are growing for case managers who can work as members of the interdisciplinary team.

CCP is the Recognized Credential for Case Managers in Chronic Care

CCP is an interdisciplinary credential that supplements specialty certifications such as certified case manager (CCM). A 2007 DMAA and HealthSciences survey of DMAA member organizations, found that CCP certification was required or recommended by nearly two-thirds of health plan and disease management organizations survey respondents.
