Individual Enrollment

An On-Line Registration Form is available for eLearning program registration. Payment may be made with Visa, Mastercard or American Express (Verisign secured), or, by check or money order.

If you would like to pay by check, please complete the online registration form, print out and mail with payment or payment information to HealthSciences Institute, 3200 N. Lake Shore Dr. #1001, Chicago, IL  60657. Or, if you prefer, you can print out, complete and send a PDF copy of the CCP Registration Form with your payment. Registration may also be sent via fax to: 866-640-6060.

Upon receipt of payment, you will receive an invoice, program link, and username/password for accessing the eLearning program. The CCP program manual and workbook are shipped via UPS Ground and will usually be delivered within three to five working days within the US or Canada. There is no charge for shipping to addresses in US or Canada. Please note: UPS will not deliver to P.O. boxes, a physical address is required.

Organization Enrollment

Contact us for more information about offering the online CCP program to your staff, including CCP as a pre-approved program for tuition reimbursement, or hosting an on site CCP event. On site delivery is available for teams of 25 or more.
