In 2004, HealthSciences Institute introduced the first professional disease management curriculum in the U.S. following its breakthrough 2003 chronic disease management competency modeling study. Now in its fourth edition, the 2008 CCP Certification program has been the cornerstone of state and organization-wide chronic care improvement initiatives.

1. What is the Value of CCP Professional Certification?

Population health improvement, disease management (DM) and health coaching are rapidly growing fields in health care. Yet, few professionals bring training or experience in DM foundations, best practices, and evidence-based approaches and interventions.

The CCP program includes a core curriculum designed to prepare case managers, disease managers and health providers to deliver improved quality and value to patients and purchasers. The program addresses not only chronic diseases, but key considerations for supporting senior health and managing age-related conditions.

CCP certification demonstrates that professional staff have mastered a core DM curriculum (defined by a third-party certification organization, designed in accordance with national standards, and based on a national competency modeling study) -- and successfully completed a certification examination.

The CCP program has been recognized by DMAA: The Care Continuum Alliance and the Case Management Society of America as the recommended credential for health and disease managers.

2. Who Should Register for This Program?

Because health and disease management are by definition multidisciplinary, consumer-focused approaches, the CCP program is designed for a range of direct service, disease management and care management professionals – including nurses, physicians, pharmacists, rehabilitation therapists, medical social workers, and allied health professionals – working in health plan, employer-based purchaser, public health, primary care, hospital, disease management, and long-term care organizations and settings. As a multidisciplinary program, there is no prerequisite professional or licensure requirement for CCP certification.

3. What is "CCP Certification?"

CCP certification is the only nationally recognized population health improvement, disease management and health coaching certification. It is designed to augment the specialized training and skills of health care providers. Unlike most certification programs the CCP program is consumer, not discipline focused. And unlike DM program certification offered by URAC, NCQA, or JCAHO certification, this program is geared toward professionals.

Individual professionals completing the program are invited to list "Chronic Care Professional Certification" when documenting professional training and certifications. Individuals completing CCP certification may also use "CCP" as a designated credential. (As always, professionals should refer to the federal, state, employer, or professional guidelines or statutes governing their profession or role to determine the specific chronic care services they may or may not provide).

Further, organizations whose professional staff have completed CCP certification may include the CCP seal on their organization web site and promotional materials. Following delivery of the program to your professionals, please contact us to request HSI and CCP logos in jpg format.

CCP certification is valid for three years. Re certification through re-examination or documentation of 15 hours of continuing education in chronic care-related areas may be obtained for an administrative fee of $75.

4. How is the CCP Program Delivered?

The CCP program can be delivered as a stand-alone offering or integrated with internal programs or existing employee development programs. Two delivery methods are available:

  • Blended Delivery – the blended program combines an intensive two-day on-site or off-site program with self-directed eLearning. The on-site program is available for groups of 25 or more. On-site delivery is a population option for larger organizations or regional groups. HealthSciences Institute also offers national pre or post conference off-site sessions for individual learners interested in participating in a live program.   • ELearning – the CCP eLearning program employs Adobe flash-based technology with audio to deliver a rich eLearning experience. The eLearning program is designed for organizations that wish to minimize travel expenses, offer just-in-time training, or reach dispersed or home-based employees. The eLearning program is also an excellent choice for individual registrants. Registrants have 12 months following enrollment to complete the program and exam. All learning is self-directed.

5. What is the Cost of the Program & What is Included?

The individual tuition fee for the CCP certification program is $1,295.00. Discounts are available for delivery of the live or eLearning program to large groups or organizations. Please contact us for more information. The program fee includes:

  • 24/7 Access to the eLearning Program. Registrants receive ongoing access to the 40-hour online CCP Certification Program for 12 months following registration from any place an internet connection is available. The only software required is Adobe Flash Player.
  • CCP Program & DM Reference Manual. This comprehensive CCP manual is designed to serve as both a learning manual and ongoing disease management reference guide. Registrants will receive the fourth edition of the manual (published in May 2008), complete with expanded patient partnering, behavior change facilitation and health coaching content--and 2008 updated evidence-based care guidelines for 25 chronic diseases and conditions of late-life. The CCP manual is the most comprehensive and up-to-date health improvement, disease management, and health coaching professional reference manual available today.
  • CCP Certification Examination. Individuals completing the CCP program are eligible to take the on-line 100 item multiple-choice certification examination. The psychometric properties of this test have been evaluated by the University of Minnesota UCCS Testing Center. A score of 70% or better is required for certification. Two attempts are allowed (a second attempt may be made after 30 days following a failed examination attempt).
  • Chronic Care Professional Certificate. Individuals will receive electronic (PDF) Chronic Care Professional (CCP) and continuing education certificates within seven to ten working days after successfully completing the CCP program and examination. Organizations whose professional staff have been certified may also request a jpg file of the CCP certification seal for use on organization web sites, promotional materials, or proposals).
  • Continuing Education Credit Certificate. This continuing nursing education activity was approved by the Wisconsin Nurses Association Continuing Education Approval Program Committee, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Upon successful completion of the program and exam, 40 contact hours will be awarded.

6. Do You Offer Discounts for Members of the Case Management Society of America (CMSA) or the Disease Management Association of America (DMAA): The Care Continuum Alliance?

HealthSciences Institute has arrangements with CMSA and DMAA to offer the CCP Certification program at a 25% discount to verified CMSA and DMAA members.

Through a special partnership, DMAA member organizations receive the following per person discounted fee based on the number of staff they register for the program.

  • 1 to 99 registrants, 25% discount ($971.25)
  • 100 to 249 registrants, 30% discount ($906.50)
  • 250 to 499 registrants, 35% discount ($841.75)
  • 500+ registrants, 40% discount ($777.00)

 

7. Do You Offer Discounts or Learning Programs for Students?

If you are a enrolled in a full-time graduate or advanced degree program, we are happy to offer a registration fee discount. If you would like to request financial assistance or are interested in an internship or other student learning program, send your request, along with your resume.

8. Is the Program Available for Delivery Outside of the U.S.?

HealthSciences Institute is currently partnering with government, university and organization leaders in international locations to offer the CCP program. To date, the program has been delivered in Canada, Europe and Asia. If you reside outside of the U.S. or Canada, payment can be made via electronic funds transfer or by international draft (in U.S. funds). Please contact us for bank routing information. International shipping via UPS is included in the fee. If you are a government, university or private organization interested in group delivery of the CCP program in your country, please contact us.

9. How Do I Register?

An On-Line Registration Form is available for eLearning program registration. Payment may be made with Visa, Mastercard or American Express (Verisign secured), or, by check or money order. (Alternatively, you can print and complete the registration form, and fax to us at 866-640-6060).

Registrants submitting credit card payment for the eLearning program will receive a link and username/password for accessing the eLearning program generally within 24 to 48 hours of registration. The CCP Program Manual and Workbook are shipped UPS Ground and will usually be delivered within seven to ten working days to US or most international locations. ELearning access and learning materials will be provided upon receipt of payment for registrants paying by check or money order.
